STATEMENT OF PURPOSE:
A Clinical Pharmacist serves as a member of Sun Life Family Health Center’s interdisciplinary care team along with doctors of medicine, physician assistants, nurse practitioners, dentists, and behavioral health consultants. As members of the interdisciplinary team in the Primary Care Medical Home model, clinical pharmacists operate under established written collaborative practice agreements with the health center’s medical director. This allows clinical pharmacists to assess patients, initiate and adjust medications, and monitor and treat disease states. Currently, collaborative practice agreements exist in the fields of Type 1 diabetes management, Type 2 diabetes management, gestational diabetes management, pain management, hypertension management, hyperlipidemia management, anticoagulation management, weight management, smoking cessation, and conducting Medicare Annual Wellness Visits. Clinical Pharmacists report to the Manager of Clinical Pharmacy Services and their respective District Lead Clinical Pharmacist.
EDUCATION AND EXPERIENCE:

• Graduate of ACPE-accredited School of Pharmacy with a PharmD degree
• Current Arizona State Board of Pharmacy License
• Experience as a Clinical Pharmacist in a similar setting preferred
