Northwest Kidney Centers seeks a Social Worker to support multiple units in a part-time, variable position.

Purpose
The Social Worker is responsible to the Social Services Manager for social work intake and patient advocacy services and for coordinating social services with physicians, NKC personnel, patients, families, and other organizations. This position is responsible for patient education and counseling, and for performing relief and/or project duties, when required. The Social Worker is part of the interdisciplinary team (IDT), and is responsible for providing each patient with an individualized comprehensive assessment of his or her social service needs, and developing a plan of care in accordance with the ESRD Conditions of Coverage.

Shift: Part-time, Variable schedule

Required qualifications

  • MSW degree
  • Current WA Social Worker License
  • Valid Washington Drivers license, reliable transportation and car insurance

Preferred qualifications

  • Previous medical social work experience
