- Works independently in the patient's home to conduct evaluations and follow-ups as needed

- Documents appropriately and timely using EHR system
- Ability to supervise Physical Therapy Assistants
- Ability to document and communicate well with the Rehabilitation team
- Reports to Director of Patient Care Services
