Torrance Memorial Physician Network is seeking an Ambulatory Care Pharmacist to add to its Care Coordination Center (CCC). The CCC is a multi-specialty clinic that addresses the needs of high-risk, complex patients of the Health System. The applicant must be a CA licensed PharmD, with PGY1 training and two years of clinic experience preferred.

Some duties and responsibilities of this position are:

• Provide coordinated, comprehensive, high-quality care to TMPN patients.
• Responsible for the evaluation, interpretation, and management of patients within the CCC together with the physician led team.
• Manages the network’s anti-coagulation population in its Coumadin Clinic at the CCC.
• Coordinates with TMHS’s in-patient hospitalist team to help oversee continuity of care for post-acute discharge patients.
• Work collaboratively on CCC workflows and protocols with the team.
• Work collaboratively with primary care physicians, specialists, staff, and leadership to develop programs that improve patient outcomes through appropriate medication therapies to improve the quality and outcomes of healthcare provided to Network patients.
• Support clinicians and other healthcare providers through collaborative drug therapy management services, medication conversion programs, and other functions to ensure that the use of pharmaceutical agents is rational and cost-effective.
• Interact with other departments and present reports and findings to TMPN leadership as needed to perform the duties of this position.
• Develop and implement methods to measure the results of these programs to justify expansion and drive quality improvement.
• Provide expertise and vision to TMPN leadership in regards to clinical and fiscal pharmacotherapy issues.

Job Type: Full-time

Pay: $150,000.00 - $175,000.00 per year
