Responsibilities

Shift: Monday - Friday 8am - 5pm Including Rotating Weekends

Pay Scale: LCSW $36.87 - $53.16; MSW $33.52 - $48.35

Job Summary:

Responsible for the assessment and delivery of counseling services to inpatients of the BHU. May provide evaluation to the Emergency Room/medical units for referred patients to BHU. Provides counseling services including assessment, diagnosis, group therapy, psychoeducation and treatment for clients/patients experiencing social/behavioral/mental health issues. This position is responsible for review and oversight of social work functions of the BHU.

Essential Duties:

  • Assesses patients' needs and accurately documents information in a timely manner demonstrating compliance with department standards.
  • Ability to quickly assess risk related to suicidal and homicidal ideation.
  • Provides oversight, monitors and evaluates the quality and appropriateness of social services furnished. The services must be furnished in accordance with accepted standards of practice and established policies and procedures. Social service staff responsibilities must include, but are not limited to, participating in discharge planning, arranging for follow-up care, and developing mechanisms for exchange of appropriate information with sources outside the hospital.
  • Completes Psychosocial Assessment within 48 hours of admission of the patient and a treatment plan that is individualized and measurable in compliance with the department and regulatory standards.
  • Liaison with courts and community mental health program for involuntary patient information and discharge planning
  • Implements discharge plan developed by multidisciplinary treatment team beginning with day of admission.
  • Schedules and coordinates after care with outpatient providers within 7 days of post discharge for the patient. This includes therapy appointments, medication management and follow up medical appointments.
  • May provides a caring contact to the discharged patient, by telephone or tele, within 48 hours of discharge and document.
  • Participates in the management of the patient milieu with other team members.
  • Provides crisis intervention to patients in immediate crisis on the unit and to recently discharged patients on an as needed basis.
  • Provides pre-admission screening/ assessment to Mercy ED and medical units
  • Facilitates psycho education groups
  • Liaison with patient family and provides family education
  • Participates in Code Gray interventions as needed.
  • Responsible for maintaining knowledge of all state and federal regulations as they apply to the admission, care, and discharge of patients from an inpatient psychiatric facility including all pertinent civil commitment procedures and protocols.
  • Performs patient screening/assessment of social, economic, emotional, and family situations.
  • Assists with responsibility for phone inquiries and patient intakes, screening and admission requests, ensuring preauthorization and utilization review as necessary.
  • Establish and maintain relationships interdepartmentally as well as within assigned departments to ensure customer and caregiver satisfaction with referrals/admissions/appointments.
  • Responsible for maintaining a thorough knowledge of community resources, state administrative rules and statutes.
  • Functions as a member of the Interdisciplinary Care Team to assure care is accomplished effectively and efficiently in a cost-effective manner.
  • Participates with team members in facilitating patient’s and family’s learning throughout the continuum of care. Reinforces patient’s continued health care through teaching and/or referral to community agency follow-up.
  • Performs patient assessment of social, economic, emotional and family situations.
  • Provides diagnostic, preventative and treatment services of a psychosocial nature pertaining to personality adjustment, behavior problems and interpersonal dysfunction.
  • Respects the privacy and protects the confidentiality of patients and family members while maintaining the boundaries of the professional discipline.
  • Provides therapy using a variety of treatment modalities, interventions and strategies.
  • Engages in active management of milieu to maintain therapeutic and safe environments.
  • Provides trauma informed care and assists with patient behavior management needing physical restraints or seclusion room.
  • Provision of unit milieu safety through patient observations, visitation monitoring and de-escalation interventions.
  • Implements intervention techniques incorporating the therapeutic concepts covered in de-escalation training and management of behaviors for patients exhibiting unsafe behavioral symptoms/behaviors.
  • Responds to emergency situations related to combative or violent people that are endangering themselves or others.

Qualifications

License/Certifications:

  • State or Oregon licensure as an MSW or LCSW required.
  • Must have current American Heart Association BLS for Health Care Providers within 30 days of hire.

Experience:

  • Two (2) years experience in evaluation and clinical intervention with a mental health population preferred.
  • Knowledge of the principles and practices of social work and patient evaluation required.
  • Must possess knowledge of community resources, state and federal social health and welfare programs and legislation.
  • Good verbal and written communications skills required.
  • Knowledge of human behavior, social systems and general social work practices and principles required.
  • Must be able to relate to a wide range of people from different cultures, professions and value systems.

Education:

  • Masters in Social Work required.
