The Social Worker works with an exemplary and robust interdisciplinary team, focused on providing quality care and support to patients in the community through education, resources, and care that honors the goals and wishes of each person.

You will be part of a close knit, highly functioning care team of social workers, chaplains, nurses, physicians, and certified nursing assistants, all highly dedicated and working together to improve people’s lives when they need help the most. You will be an advocate by providing education and resources to patients and their families.

The Social Worker will have two distinct roles: advocating for hospice patients and their families and providing support to patients and their families as they negotiate the emotional challenges surrounding end of life in a way that honors the goals and wishes of each person.

Requirements, Pay and Benefits:

• Bachelor's Degree in Social Work from an accredited social work program required.
• HopeWest will support you in obtaining your CSWC or LCSW If you have a master’s in social work including providing social work supervision while employed.
• Minimum of one-year supervised experience in the health care field or during a supervised internship.
• Participates in the on-call, holiday, weekend, and after-hour’s rotation.
• Effectively communicate with others, both written and verbal
• $27 - $31/Hour, depending on experience and education, plus any shift differentials that may apply.
• Premium pay for Licensed Clinical Social Worker and/or bilingual fluency

The Job Location:
Across Delta county: HopeWest Social Workers provide care in patients' home settings. When a patient lives in a nursing home or assisted living facilities, we partner with professional colleagues in those settings to provide care.
