The Intake Nurse will be responsible for initiating and processing patient referrals to assure that patient needs are met and agency policies are followed. Establishes and maintains ongoing relationships with referral sources, payors and case managers.

Education: Completion of an accredited RN program. BSN preferred

Required Licensure, Certification: Maryland Licensure as a RN

Work Experience

  • 3 years of clinical nursing experience.
  • 1 year work experience in a position of responsibility (administration/ supervision/ preceptor) preferable in a case management, community health or home health setting
  • Home medical equipment and infusion experience preferred.

Tools & Equipment

  • Office and telecommunications equipment
  • EPIC experience is a preference
