Wahiawā Center for Community Health is a FQHC- Look-a-Like health center in Wahiawa. We are committed to ensuring the delivery of quality care through continuous evidence-based improvements. All employees are a part of PCMH and are required to actively participate in quality initiatives, including trainings and evaluation projects. Employees are expected to demonstrate and promote team-based and patient centered interactions with other staff members and patients/members/clients.
We are looking for a Community Pharmacist that will provide comprehensive pharmacy services to patients and support clinic departments to ensure that medications are dispensed in a safe, efficient and accurate way. Community pharmacist will work collaboratively with healthcare professionals to enhance patient safety and ensure that Medication Management practices are safe and effective.
Duties and Responsibilities include but not limited to:
Pharmacy Support Services:
1. Process, fill and dispense prescriptions: Verify safety and accuracy of prescriptions, verify patient data and billing information to prevent and/ or resolve billing errors, accurately interpret prescriptions from prescribers to properly dispense medications and health products.
2. Monitors and assesses 340B guidance and/or rule changes, including, but not limited to, HRSA/OPA rules and Medicaid changes to ensures that the 340B pharmacy program is continuously compliant with 340B federal regulations.
3. Support clinical departments to ensure that patients receive their medication in a timely, professional and efficient manner with all the necessary practical and clinical information
Wahiawa Health Community pharmacist will also provide a wide range of services including educational consultations, medication management and other medication optimization services, chronic condition management, diabetes education, tobacco cessation and diabetes prevention services, immunizations, patient empowerment, health and wellness services, and other services that help to improve the lives of patients in the community
EDUCATION/EXPERIENCE:
1. Doctor of Pharmacy Degree from a school recognized by American Council of Pharmaceutical Education (ACPE)
2. PGY1 residency is a plus OR a licensed Pharmacist who have not completed a PGY1 residency but demonstrates mastery of the knowledge, skills and attitudes and abilities expected of who has completed a PGY1 residency
3. Must Complete 340 B pharmacy drug pricing program training when hired
4. Minimum of one year pharmacy experience in a hospital or health care center setting is preferred but not required
CERTIFICATION/LICENSURE :
1. Hawaii Pharmacist License
2. Board Certified Ambulatory Care Pharmacist (BC-ACP), CDE or BC-ADM is a plus
3. Immunization Certificate required
4. Must be willing to take CDE exam after one year of hire.
OTHER REQUIREMENTS: Must be physically capable of standing and/or sitting for extended periods of time and physically capable of lifting and carrying to 10 pounds.
